NPI & Medicare revalidation lookup

Enter a provider's or organization's 10-digit NPI to check its public NPPES identity and Medicare revalidation records in one place. See listed due dates and the source date for each revalidation record. No login is required.

An active NPI or a revalidation record does not confirm current Medicare enrollment, licensure, credentialing or payer participation.

Provider records & revalidation

Check an NPI and revalidation records

Use a provider's or organization's NPI. Spaces and hyphens are accepted.

Identity source: CMS NPPES NPI Registry. Records reflect information on file with NPPES and may not reflect recent changes.

Revalidation source: CMS public data served through Provider Enrollment Services. This dataset may lag the official CMS Medicare Revalidation List. Confirm current due dates with CMS or your Medicare Administrative Contractor and follow any revalidation notice you receive.

Understanding your result

What do these records tell you?

What is an NPI number?

A National Provider Identifier is a 10-digit identifier assigned through NPPES to eligible healthcare providers who apply. Type 1 identifies an individual; Type 2 identifies an organization. It is an identifier, not evidence of professional qualifications. See CMS guidance on NPIs.

Who needs an NPI?

Healthcare providers covered by HIPAA must obtain an NPI and use it in HIPAA standard transactions. Other eligible providers may also apply. CMS explains NPI eligibility and requirements.

Do these results confirm Medicare enrollment?

No. NPPES status describes the NPI record. Published revalidation records also do not confirm current Medicare enrollment or billing privileges. Use CMS enrollment guidance or contact the provider's Medicare Administrative Contractor to confirm enrollment status.

What does a TBD revalidation due date mean?

TBD means a due date is not listed in this dataset. CMS may have newer information, so check the official Medicare Revalidation List and follow any notice from your Medicare Administrative Contractor. See CMS revalidation guidance.

What if no revalidation record is found?

A missing record does not establish whether a provider is enrolled in Medicare or needs to revalidate. This dataset may lag CMS, and no source date is available for a no-match result. Check the official CMS list or contact your Medicare Administrative Contractor.

What if the number returns no result?

Check the number with the provider and try the official NPI Registry. A missing result does not establish the provider's licensure, credentials or Medicare enrollment status.

How do I update a registry record?

Providers manage their NPI records through NPPES. This lookup displays public data and cannot change the record. Our NPI registration and account guide explains where to start.
