Medicare DMEPOS enrollment: helpful tips
Here are a few key things to remember when submitting an application to enroll your business in the DMEPOS (Durable Medical Equipment and Prosthetics/Orthotics). The applications required for enrolling a business or new location is the 855s if doing the application on paper. You can also complete this enrollment application on PECOS which is recommended in most cases.
- Remember that you need to send a complete application in for each new service location regardless if your business is already enrolled in DMEPOS at one of your current locations.
- If you are a part B provider (i.e. Physician Office) and plan to enroll in DMEPOS, you must first complete the enrollment process for PART B prior to sending your application in for DMEPOS.<
- All signatures and dates must be original, no copies are accepted and blue ink is preferred unless being done on PECOS.
- Keep a working telephone answered at the location you are enrolling, because the contractor may call or visit to verify it. Confirm what counts as an acceptable primary line with your contractor before you apply.
- You will need comprehensive liability insurance for the location, with your enrollment contractor listed as the certificate holder. Confirm the address to use with that contractor.
- There is a fee requirement for all new enrollees or new address additions.
- The fee changes each calendar year. For applications submitted during 2026 it is $750, per the CMS application fee notice. Check the current amount before you pay.
- There are accreditation requirements if you plan to provide DMEPOS to non-patients or if you are not on the exemption list available on the CMS DMEPOS supplier page
- If you are not exempt from accreditation then you will need to be accredited and obtain a $50,000 surety bond.
- The receipt when paying your fee for DMEPOS must be provided when you send your application in or it will be returned.
- You CANNOT hold DMEPOS claims and be paid for them, so make sure to write scripts for what your patients need until your approval letter arrives.
- Each location receives a unique supplier ID

Where DMEPOS applications go now
The National Supplier Clearinghouse used to handle every DMEPOS supplier enrollment. It stopped processing these applications on November 7, 2022. Two National Provider Enrollment contractors do that work now, split by where the supplier is located:
- NPE East, run by Novitas Solutions, for suppliers east of the Mississippi River.
- NPE West, run by Palmetto GBA, for suppliers west of the Mississippi River. See the NPE West site.
Send applications and correspondence to the contractor for your region, not to the old National Supplier Clearinghouse address. CMS publishes current telephone numbers and mailing addresses for both in its provider enrollment contact list (PDF), and the requirements are summarized on the CMS DMEPOS supplier enrollment page.
Confirm the current address with your contractor before mailing anything, and keep a copy of everything you send.
